A 60y/M with CKD



 This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. This E log book also reflects my patient-centered online learning portfolio and your valuable inputs on the comment box.

26NOV 2021

A 60Y/M came to OPD with 

C/O pedal edema since 2-3yrs

C/O Decreased urine output since 2-3yrs

History of present illness : 

A 60Y/M Businessman (cattle bussiness)by occupation whose daily routine 20yrs back was to roam around the town and sell cattles until he started having pedal edema, decreased urine output , he then went to hyd where he was adviced some medication and his complaints got relieved 

There were no complaints until 2-3 yrs back where he again had the same problem of pedal edema, decreased urine output , SOB grade 2-3 .He was taken to a hospital where he was prescribed some medications(?Lasix). Pedal edema decreased and his urine output improved.

Again one year back he started having similar complaints(pedal edema, decreased urine output ) he was taken to nims where he was adviced dialysis and since then he was undergoing dialysis.

2 per week from 8-9 months.


Past history :

Patient is a known case of hypertension 

Patient is not a known case of Diabetes mellitus 


Family history : 

No relevant family history


General examination :

Patient is concious , coherent,cooperative and well oriented to time and place 

Bp:130/80

Pr:90

temp :98.6F

spo2-98% at RA


Systemic examination:

CVS: 

S1S2 Heard 

no thrills and murmurs 


RS: 

BAE+

NVBS+ 


Per abdomen ;

Soft,non tender 

no palpable mass 


CNS : NFND


Provisional diagnosis : CKD on MHD
